Definition of handover form

A handover, assignment or checkout form serves as a official document of handover, delivery, assignment or acceptance of something between two parties - the transferor and the transferee.

The checkout or hand over  form serves to confirm the act of handover itself - that is, that the transferor has handed over the item to the recipient and that the recipient has accepted the item. It may also serve to record the condition of the object of the transfer.
