A printable medical declaration form makes it easy for you to automatically create and print declarations for your employees. It is run from the employee record and will automatically populate the employee's employment file. You just print the medical declaration, hand it to the employee and they sign it.
The signed and scanned declaration can then be uploaded to your personnel file by you as HR manager or the employee themselves.
