An onboarding questionnaire (also called a New Hire Survey) is a short form that a new employee completes before their start date or during their first few days. It helps the company (HR and the hiring manager) quickly collect key information—contact details, preferred name, equipment requests, system access needs, required training, and any other support—so onboarding can be prepared efficiently without unnecessary back-and-forth emails.
How to Use the New Hire Onboarding Questionnaire Template
- Open the Employees organizer
- Go to the orghanizer settings
- Select the "Online Forms" tab
- Choose a template from the online forms library
- Publish the onboarding questionnaire
